Risk Identification Survey
Domestic Abuse is a crime which affects individuals and families from all backgrounds. It can be difficult to identify and even harder to accept, especially for the person experiencing it. People sometimes mistake it as only physical violence when actually it includes many more behaviours that are perpetrated to exercise control and power.
This risk assessment survey is used by front-line specialists, like us, to identify high risk cases of domestic abuse. Its sole purpose is to safeguard your safety and protection.
To complete each question, select yes or no then use the comment box provided to give as much detail as you can. Upon completion, you will be given the choice to share your results with us so we can work with you to get the help you need.

You can register 999 on your phone so you can text 999 in an emergency instead of calling. This might be a safer way to contact the police, ambulance or fire during an emergency if violence is happening at home.
To register, text ‘register’ to 999. You will get a reply – then follow the instructions you’re sent.
In an emergency Text 999. In the text message write:
Who? Police, Fire, Ambulance
What? Briefly, what is the problem?
Where? Exactly where the problem is happening – give the name of the road, house number, postcode or nearby landmark if possible.
The emergency service will either ask for more information or will tell you that help is on the way. Don’t assume that your message has been received until the emergency service sends a message back. It will usually take about 2 minutes before you get a reply. If you don’t get a reply within 3 minutes, please try again or find other ways of getting help.
